Housing Plus Project

The Housing Plus Project (HPP) provides housing assistance and related supportive services to help clients achieve long-term stable housing and engagement and retention in HIV care. Resources related to the HPP will be included on this page and updated frequently.   

Required Provider Forms

Providers are required to utilize standardized forms for collecting client information. Providers must maintain copies of completed HPP forms for each client. Providers may keep these in a paper client chart or upload them to an electronic medical record system or ARIES. Please see the most recent versions of the provider forms below.
