HIV Care Program (HCP)

Clinical Quality Management (CQM)

The California Department of Public Health (CDPH), Office of AIDS (OA), Health Resources and Services Administration (HRSA), Ryan White HIV/AIDS Program (RWHAP) Part B, CQM program coordinates activities aimed at improving care, health outcomes, and satisfaction for Californians served by the RWHAP Part B grant. CQM activities focus on programs and services funded by RWHAP Part B, which includes OA’s HCP, AIDS Drug Assistance Program (ADAP), Minority AIDS Initiative (MAI), and Emerging Communities (EC).

HCP sub-recipients who provide RWHAP Part B services also participate in the CQM program. HCP sub-recipients include HCP-funded agencies that are critical to the success of the CQM program. HCP sub-recipients are generally local health jurisdictions or community-based organizations contracted to provide a range of allowable HIV core medical and supportive services.

HCP CQM Performance Measurement Tools

The HCP CQM performance measurement tools were developed to calculate performance measures. Performance measurement is required by HRSA CQM Policy Clarification Notice 15-02 and the OA CQM Plan.

The tools include one “HCP CQM Performance Measures” Workbook and five ARIES reports that work together to calculate performance measures for four primary services within RWHAP Part B programs (Outpatient/Ambulatory Health Services Medical Case Management Services, Non-Medical Case Management Services, and Food Bank/Home-Delivered Meals).

HCP sub-recipients can use these tools to calculate performance measures and to identify and follow-up with clients who do not meet the performance measures.
